Overview

Breast augmentation is the placement of breast implants to enhance the shape or volume of the breast according to the patient’s goals.

The goals of a breast augmentation are proportion, symmetry, and a natural appearance in clothing and out. The best outcomes are those in which the breasts appear harmonious with the rest of the body — a size and shape that suits the patient’s frame, matches her aesthetic, and tailored to her individual anatomy.

Every breast augmentation involves several decisions: the type of implant (saline or silicone), the size and profile of the implant, the position of the implant (above or below the muscle), and the location of the incision. These decisions are made together during a careful consultation.

Who is a Candidate

The best candidates for breast augmentation are patients in good health who are bothered by the size or shape of their breasts. Common motivations include small breast size, asymmetry between the two sides, loss of volume after pregnancy or weight change, or a desire for increased fullness.

Candidates should be at least 22 years of age for silicone implants, per FDA guidelines.

Breast augmentation addresses volume and shape, not skin quality or significant sagging. Patients with substantial breast laxity may benefit from a breast lift, with or without an implant.

What to Expect

Every case begins with an unhurried consultation. Dr. Nissan will examine the breasts and chest wall, discuss what you notice and what you hope to change, and review implant options in detail. Sizers or three-dimensional imaging may be used to help visualize potential outcomes.

Surgery is performed under general anesthesia as an outpatient procedure. Discomfort is minimal and typically easily managed with non-narcotic pain medications. Most patients are comfortable returning to desk work within a week or earlier, and full activity within four to six weeks. The final position and shape of the breasts continues to settle over three to six months.

Saline or silicone: which is better?

Both are safe, FDA-approved options with distinct characteristics. Silicone implants feel more like natural breast tissue and are the more common choice today. Saline implants are filled after placement, allowing for slightly smaller incisions, and any deflation is immediately apparent. Dr. Nissan will discuss which is right for you based on your anatomy, goals, and preferences. He typically recommends saline implants.

Above or below the muscle?

Both positions have advantages. Below-the-muscle (dual-plane) placement typically offers a more natural upper-pole slope and reduces the visibility of implant edges. Above-the-muscle placement can be preferable in specific anatomic situations. Dr. Nissan will recommend the approach best suited to your anatomy.

Where will the scar be?

Incisions may be placed in the crease under the breast (inframammary), around the areola (periareolar), or in the armpit (transaxillary). Each has trade-offs. Dr. Nissan will discuss which incision is right for your case and preferences.

How long do implants last?

Modern silicone implants are engineered to last decades, but they are not considered lifetime devices. The FDA recommends routine imaging to monitor implant integrity over time, typically five years after surgery and every two to three years after that. Patients should expect that they may desire implant exchange or removal at some point in their lifetime.

Will I still be able to breastfeed?

Breast augmentation is generally compatible with future breastfeeding, particularly when the incision is placed in the inframammary crease. That said, breastfeeding ability is variable in the general population and cannot be guaranteed.

How soon can I return to activity?

Most patients are back to desk work within a week. Light activity resumes within two to three weeks, and full activity — including exercise — within four to six weeks. The exact timeline depends on the individual and the specifics of the operation.

What happens at the consultation?

The consultation is a conversation as much as an examination. We will take your photos, and Dr. Nissan will examine the breasts and chest wall. He will discuss implant options, sizes, and placements, and recommend the treatment strategy that best serves your goals.
