Nose
A refined, natural approach to reshaping the nose in harmony with the rest of the face.
Rhinoplasty is the surgical reshaping of the nose.
The goal of a rhinoplasty is a nose that appears natural, unoperated, and in harmony with the rest of the face. The best outcomes are those that make the patient look like a more balanced version of themselves — not a change of identity, not an unnatural-appearing nose imposed onto their features, but a refined version of the nose they were born with.
Rhinoplasty can address a wide range of concerns: a dorsal hump, a bulbous or drooping tip, a wide or asymmetric nose, and breathing difficulties. Every rhinoplasty is customized to the patient’s anatomy and goals.
The best candidates for rhinoplasty are patients in good health who are bothered by a specific feature of their nose and have realistic expectations about what surgery can and cannot accomplish. Common concerns include a dorsal hump, a bulbous or droopy tip, a wide bridge, asymmetry, or difficulty breathing through the nose.
Rhinoplasty is deferred until facial growth is complete — around age 16 for females and age 18 for males. There is no upper age limit; adults in their sixties and beyond may be candidates provided they are in good general health.
Rhinoplasty is a customized operation. What is right for one patient’s nose is not right for another. A careful consultation and detailed discussion of goals is essential.
Every case begins with an unhurried consultation. Dr. Nissan will examine the nose from all angles, including internal examination of the breathing passages, and discuss what you notice and what you hope to change. Digital imaging may be used to help visualize potential outcomes.
Surgery is performed under general anesthesia. Bruising around the eyes is common in the first week and resolves substantially by two to three weeks. A small external splint is worn for the first week. Most patients are comfortable returning to social settings by one to two weeks, although subtle swelling of the tip continues to refine over a full year.
Do you perform preservation rhinoplasty?
Dr. Nissan is trained in preservation rhinoplasty as well as traditional structural rhinoplasty techniques, and selects the approach that best serves each patient’s anatomy and goals. He has published on preservation rhinoplasty in the Aesthetic Surgery Journal.
What is the difference between open and closed rhinoplasty?
Open rhinoplasty involves a small incision across the columella (the strip of tissue between the nostrils), allowing direct visualization of the entire nasal framework. Closed rhinoplasty is performed entirely through incisions inside the nose. Both are legitimate approaches, and each is preferred in different scenarios. Dr. Nissan performs both and selects the approach based on the specific goals of the operation.
How long does the result of rhinoplasty last?
The result of rhinoplasty is permanent. The nose will continue to age like the rest of the face — cartilage and skin change gradually over decades — but the underlying structural changes made during surgery are lasting.
When can I see the final result?
The final result of rhinoplasty takes time to appear. Bruising and major swelling resolve within two to three weeks. Subtle refinement of the tip continues over a full year, and sometimes longer, as the last remaining swelling resolves. Patience is an essential part of the recovery.
Is rhinoplasty painful?
Most patients are surprised by how little pain they experience after rhinoplasty. Congestion and pressure are more common complaints than pain. Discomfort is managed with a short course of medication in the first several days.
Can rhinoplasty improve my breathing?
Yes. Functional concerns such as deviated septum and turbinate hypertrophy are frequently addressed at the time of aesthetic rhinoplasty. Every rhinoplasty should result in a beautiful nose that breathes well.
What happens at the consultation?
The consultation is a conversation as much as an examination. We will take your photos, and Dr. Nissan will examine the nose from all angles, including the interior. He will discuss the underlying anatomy driving your observations and recommend the treatment strategy that best serves your goals.
